Holocaust Services Geriatric Case Manager

Job Overview

Ruth and Norman Rales Jewish Family Services is currently hiring a full time Holocaust Service Geriatric Case Manager. This position is responsible for assesing individuals and couples capabilities to remain safely at home or assist them in developing alternate living arrangements. The Case manager will assist in gaining access to needed medical, social, educational, physiological and other essential services. The case manager will asses the clients' strengths and limitations and develop a plan of care. Case manager will coordinate services, act as a linkage to community resources and monitor overall service delivery. Case manager will effectively communicate with service providers and family members. 

Required Experiance and Education

• Master's in Social Work
• LCSW prefered
• Minimum 2-5 years of experience in geriatric case management
• Effective verbal and written communication skills
• Demonstrates ability to resolve problems and provide assistance and teamwork where appropriate.
• Strong organizational skills as reflected in effective scheduling, planning and meeting deadlines.
• Requires flexibility with scheduling on an as needed basis.
