Case Manager - Kindness Initiative

About the Kindness Initiative:

The Kindness Initiative is a 501(c)3 nonprofit organization dedicated to serving individuals and families in the San Diego Jewish community who are navigating the challenges of poverty.

Rooted deeply in Jewish values of compassion, community, and dignity, we provide a customized and culturally sensitive case management approach. Our goal is to offer support for our "Members" (those we serve) on their journey to self-sustainability.

MISSION: Our mission is to alleviate poverty in San Diego's Jewish community by mitigating individual crises and fostering self-sustainability and personal dignity. We pride ourselves on a unique concierge service model, offering wraparound case management, supportive programs, and vital connections to community resources.

Position Summary:

As a Case Manager at the Kindness Initiative, you'll be a vital source of support for our Members. You will provide comprehensive assessments, expert resource guidance, and continuous, compassionate, and culturally competent care. This role is central to our mission, involving conducting intakes, developing individualized service plans, coordinating essential resources, and advocating passionately for our Members. You'll also collaborate closely with our dedicated KI "Angel" Volunteers and community partners to empower Members on their path to self-sustainability.

Key Responsibilities:

Member Support and Case Management

  • Conduct thorough intake calls and biopsychosocial assessments to understand each Member's unique needs.
  • Develop and monitor individualized service plans with clear, measurable (SMART) goals and relevant resources.
  • Perform regular re-assessments to ensure continued Member eligibility and progress.
  • Provide ongoing wraparound case management services by actively collaborating with external organizations and community resources.
  • Foster strong, problem-solving relationships between Members, support staff, and KI "Angel" Volunteers. Serve as a dedicated advocate for Members with various community organizations and service providers.
  • Facilitate Member support groups or workshops to empower Members, offering an exciting opportunity to lead impactful programming. These activities may be required periodically as part of case management duties to support Member growth and community engagement
  • Consistently treat all Members with the utmost dignity, compassion, and understanding.

Program Support and Documentation

  • Maintain accurate and timely records of Member information and interactions within designated systems.
  • Ensure efficient service delivery by communicating effectively with all support staff assigned to a Member.
  • Collaborate with KI "Angel" Volunteers, coordinating support activities, providing guidance, and ensuring alignment with Member service plans.
  • Uphold the strictest confidentiality of Member information and records, adhering to all KI protocols.
  • Actively assist in expanding KI's resource network to meet the diverse and evolving needs of our Members.
  • Collaborate with the Kindness G'MACH for appropriate cross-referral opportunities.
  • Contribute valuable input as part of our continuous organizational process improvement program.
  • Participate in weekly and monthly case management team meetings.

Qualifications:

Education and Experience

  • Required: Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
  • Preferred: Master's degree.
  • 2-5 years of relevant human services experience, including comprehensive case management, assessment, service planning, coordination, and documentation.
  • Proven experience in de-escalation and crisis intervention.
  • Experience in client advocacy, outreach, and resource coordination.
  • Demonstrated knowledge of and an established network of community resources addressing diverse Member needs. Skills and Competencies
  • A strong personal drive and commitment to alleviating poverty within the San Diego Jewish community.
  • Possess a genuinely kind, compassionate, and patient personality.
  • Knowledge of and sensitivity to Jewish culture and familiarity with the San Diego Jewish community is preferred.
  • Ability to maintain clear and professional boundaries with Members.
  • Exceptional independent judgment and solutions-oriented outlook.
  • Effective time management skills and accountability.
  • Excellent organizational skills with the ability to prioritize tasks, complete work efficiently, and maintain accurate, orderly, and timely records.
  • A strong ability to discuss sensitive matters in a non-judgmental manner and respect strict confidentiality.
  • Proven experience working effectively with people from diverse demographics.
  • Familiarity with basic budgeting and financial coaching principles.
  • Upholds the highest standards of confidentiality, integrity, and respect for others.
  • Skilled at promoting program services and networking with partner organizations.
  • A strong collaborative team approach.
  • Flexibility to accommodate Member needs as they arise, within reason.
  • Proficiency in Microsoft Office applications (Outlook, Teams, Planner, Word, Excel) and comfort with case management software (training provided as needed).
  • Bilingual skills in Hebrew, Yiddish, or Spanish are a plus but not required.
  • Ability to thrive in a fast-paced, dynamic, and mission-driven environment.

Position Structure

  • Reports to: Case Management Program Director
  • Hours: 32-40 hours/week
  • Work Location: This is a full-time, in-office role requiring regular on-site presence. The position involves occasional off-site meetings and approximately 10% travel to Member locations. Limited flexibility for remote work may be available, subject to KI needs.

Candidates must reside in or be willing to relocate to the San Diego area before starting the position.

  • Work Environment: This role involves emotionally demanding work supporting individuals in crisis. The Kindness Initiative provides regular supervision and robust team support to all staff.
  • Salary: Paid on an hourly basis, commensurate with experience. Starting salary of $27- $30 per hour, with opportunities for growth.
