Job Description

 

Position: Long-Term Care Social Worker    

Location: Atlanta, GA                       

Department: Social Services

Reports to: Director of Social Work Services                      

FLSA Status: Exempt

        

Summary: The long-term care social worker reports to the Director of Social Work Services.  (S)he is responsible for providing social work services to residents, their families and responsible parties.  (S)he is responsible for social work, resident care planning and related documentation.  When necessary, the social worker will assist with the responsibilities and duties of the Director of Social Work Services.                                               

Qualifications:

 Bachelor’s in Social Work (BSW) required.

  1. Master's in Social Work (MSW) preferred.
  2. Knowledge of working with the elderly preferred.
  3. Knowledge of Jewish values and customs preferred.
  4. Proficient computer skills.

  Responsibilities:

 Performs all long-term care resident MDS assessments

  1. Schedules and participates in long-term care resident care plan meetings
  2. Attends to psychosocial needs of long-term care residents and their families
  3. Document and update Code Status for Long-term patients – Do Not Resuscitate Order or Full-Code
  4. Calls families/responsible parties regarding residents' needs
  5. Mediates between resident/staff and resident/resident.
  6. Attend interdisciplinary update meetings, as needed.
  7. Attends all federally mandated staff meetings
  8. Participate in monthly and quarterly Quality Assurance meetings, as needed.
  9. Facilitate/coordinate medical, dental, eye, podiatry and other services for residents
  10. Facilitates enrollment in partner programs for long-term care residents including Optum
  11. Completes all required documentation for long-term care residents including updating electronic medical record with care plan notes, assessments, and documenting patient and family interactions
  12. Facilitates and coordinates all required forms and paperwork for Medicare compliance for long-term care residents
  13. Cross train daily responsibilities with Director of Social Work Services.
  14. Facilitates new move ins to long-term care, provide resources and support to new families
  15. Provide programming consistent with JHL core values
  16. Other duties as delegated by supervisor.

Physical Requirements:  Ability to lift up to 30 pounds; ability to sit for 80% of the day; ability to stand for long periods of time as needed

 

Mental Requirements:   Ability to focus for extensive period of time on data, letter writing and other administrative duties.  Requires mental acuity and ability to interact with residents, family and staff.
