JF&CS is now seeking a full-time Supportive Services Manager position.
 
PURPOSE: To manage and oversee services provided for family caregivers and individuals through the Caregiver Support grant funded programs and the Geriatric Care Management Services.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
  • Supervise team of professionals (case managers/geriatric care managers) providing supportive services to elders and family caregivers
  • Work with lead case manager to manage grant funded case management programs
  • Maintain and track demographics, outcomes, funding and financials for all programs; monthly, quarterly and/or yearly, as needed
  • Manage programs’ controllable cost budget
  • Supervise case manager’s caseloads, ensure proper and appropriate services are administered
  • Outreach using partnerships to enroll low-income individuals and caregivers in case management support programs
  • Ability to establish processes and systems to standardize assessment data collection, progress note taking and service delivery for both case management and geriatric care management services
  • Provide geriatric care management/case management services to individuals, as needed. Conduct client assessment, develop care plan based on assessment results, and follow up with/reassess clients as required by grant/contract and/or as needed
  • Maintain all documents and client files in accordance with CARF (the agency’s accrediting body) regulations and funding requirements
  • Oversee the CARF certification process in 2020 for the Supportive Services Team (GCM CARF)
  • Enter and submit all required information and documentation in Electronic Health Records software (NextStep)
  • Participate in supervision with all supervisees and team meetings
  • Attend required staff meetings and professional development trainings and workshops
  • Oversee annual caregiver retreat
  • Supervise facilitator of support groups and other programming
  • Assist with outreach and marketing to inform referral sources about revenue generating services and other Aviv services available for elders and their caregivers through JF&CS
  • Create and manage related program paperwork and reporting, as required
  • Assist Aviv Director in content for grant proposals and with creation of new programs and partnerships
  • Work with a lead case manager to manage the grant funded/case management programs
  • Oversee the Respite Voucher Program (ARC Respite Grant for Caregivers)
  • Represent the Supportive Services team with grant funders for various case management programs
  • Oversee and track all demographic and outcome reporting information for all supportive services (Case Management/Grant Funded services, GCM private pay and federation funded, Support Groups, Education, BHBC, Medicaid Assistance Program, and the Voucher Respite Program)
  • Administer surveys for all Supportive Services programs and analyze results for reporting purposes

  • Supervisory experience and experience with program management
  • Knowledge of community resources for the caregiver and elder community; knowledge of entitlement programs considered a plus

  • Comfort level with evaluation, budgeting, and planning
  • A good team player
  • Able to plan and help clients reach goals
  • Good use of the English language and able to communicate clearly
  • Must be able to manage time effectively, maintain boundaries and cope with crises
  • Must have valid Georgia Driver’s License and insured personal vehicle
  • Other duties as assigned
EDUCATION and/or EXPERIENCE:
  • Master’s degree in an area of human services preferred, with related experience working with elders and family caregivers, as well as strong knowledge of community resources.
  • Candidate must have experience supervising others and managing programs.
