Duties and Responsibilities:
The Care Coordinator/Case Manager will be responsible for ensuring holistic care coordination and tracking of patient care to achieve treatment goals. Specifically, s/he will ensure that consumers receive clinical and support services including therapy, case management, family counseling, peer support, appropriate referrals and whatever additional services are needed to support recovery and independence (housing, food, transportation etc.). S/he will also oversee all workflows and care navigation processes including consents, as well as care navigation and data exchange with community partners.
Project will include ability to implement this approach of service delivery within a comprehensive treatment model that includes direct services, referrals and case management of mental health and substance use.
Special Requirements: Professional with at least 2-3 years of experience in case management and clinic work, preferred.
