This role presents an exciting opportunity for a compassionate and skilled Registered Nurse to join a mission-driven team committed to delivering high-quality, home-based healthcare services. As a key contributor to a dynamic field nursing team, you will directly impact the well-being of patients while enjoying the flexibility and support of a collaborative work environment. This position blends patient-centered home visits with professional in-office coordination and care planning.

Key Responsibilities

  • Conduct initial assessments for new clients to evaluate healthcare needs and establish individualized care plans.
  • Perform ongoing home visits to monitor client progress and revise care strategies as needed.
  • Collaborate with interdisciplinary teams to ensure consistent and comprehensive patient care.
  • Provide education and emotional support to clients and their families to promote health and wellness.
  • Maintain accurate and timely documentation in compliance with regulatory standards.
  • Lead in-service training sessions for Home Health Aides and Personal Care Assistants to reinforce care quality.
  • Participate in team meetings and contribute to continuous improvement initiatives.

Qualifications & Experience

  • Active and unencumbered RN license in the State of New York.
  • Minimum of one year of nursing experience, preferably in home health or community-based care.
  • Proficient in clinical documentation and care coordination practices.
  • Valid driver’s license and access to reliable transportation.

Preferred Qualifications

  • Prior experience conducting home health assessments and supervising support staff is a plus.
  • Familiarity with EMR systems and care planning protocols.

Work Environment & Benefits
