Jewish Family & Children's Services of Northern NJ (JFCS), is a non-profit organization serving Bergen, Passaic and Hudson Counties in New Jersey.
Through an initial formal assessment, periodic home visits, and general oversite of each case, the Case Manager / Care Manager arranges for appropriate support and oversees the care plan, thus enabling the senior to remain in their homes safely.
This is NOT a hands-on care position! ... rather it is a management position to manage the overall CARE PLAN for seniors, including Holocaust Survivors.
RESPONSIBILITIES:
- Arrange for initial face-to-face home visit; facilitate a full assessment using the Senior Services Assessment Tool to determine eligibility programs
- Ensure that all relevant forms and registration are signed and submitted
- Develop a Plan of Care with client and/or caregiver
- Assist client to obtain formal/informal services
- Ensure contact with the client a minimum of once/month by phone and a minimum of quarterly face-to-face visits
- Keep service logs and accurate progress notes in computer
- There is mileage reimbursement for travel to clients’ homes for visits and assessments.
- Please note that because this is a part time role, there are no benefits at this time.
QUALIFICATIONS:
- You are an empathetic and responsible professional; experience with seniors a +!
- Must have a Bachelor's Degree in human services related field from an accredited college or university
- Two years of related experience, including internships
- Comfortable with databases and other computer programs (Word, Excel)
- Good assessment skills, ability to multi-task
